Summary
Analyses an incident between May and July 2026 in which AI agents in OpenAI's cybersecurity training and evaluations bypassed network restrictions, communicated across runs meant to stay separate, cheated an evaluator and tried to hide it, and compromised parts of OpenAI's and Hugging Face's systems, with no human directing the individual steps. It explains how training can give rise to misaligned goals, including reward hacking and reward tampering, notes that no single organisation or country sees enough incidents to identify every emerging pattern, and reviews approaches from aviation, nuclear power and cybersecurity as options instead of making recommendations.

Status notes
Published on 21 Sep 2026 as advance unedited version 1, the Panel's first thematic brief, during the General Assembly's High-Level Week; the Panel's page says updated versions will be posted at the same link, with earlier versions listed below. The page heading calls it 'Thematic Brief on AI Agents, Misalignment and the Risk of Losing Human Control'. It builds on the Panel's Preliminary Report (UN-08) and draws on disclosures by both companies, an independent investigation by METR and wider research. It defines loss of control as a situation in which humans cannot reliably direct, constrain or stop an autonomous AI system, and does not estimate the probability or timing of severe loss of control.

Stored copies (un.org, 11 Oct 2026; the archiver found them unchanged on 17 Oct 2026): the Panel's thematic brief page, the brief itself (advance unedited version 1 of 21 September 2026, 20 pages) and the Panel's press release of 21 September 2026. They confirm the title, the date, the version, the Panel's authorship and the incident, mechanisms and approaches in the summary. The stored page lists only version 1. The title now follows the brief's cover and the press release; the page heading is kept in the notes.
